POLICIES RULES AND REGULATIONS 

Bedazzle Dance Studio Inc. hereinafter referred to as (BDS, Bedazzle Dance Studio, Bedazzle Dance Studio Inc., Studio) POLICIES RULES AND REGULATIONS “Policies”, “Rules”, “Policy” “PRR”
This policy which may be updated from time to time without notice to you. You can review the most current version of Bedazzle Dance Studios PRR any time at: http://bedazzledance.com/pages/POLOCIESnew1.htm and the most recent version of the Policy Rules and Regulations at the URLs indicated above. You may also request a free copy of the PRR at our studio located at 458 Midland Ave Staten Island NY, 10306
TUITION- Sept through June Session: a monthly fee paid in 10 (ten) monthly installments
REHEARSAL&RECITAL FEE- one time fee of $20 for one student and $30 for a family. Fee is due in the month of March.
Summer Session: 1 payment for summer session. 1 month program

Summer Session – 1 or 2 days a week, 4 week session. Summer classes may combined ages, if classes have small enrollment. All summer session dance classes must be paid in advance in full.

Payment is due by the 5th of the month..
IF YOU PAY AFTER THE 5TH OF THE MONTH, THERE WILL BE A LATE FEE OF $10.00 ADDED TO YOUR ACCOUNT EACH MONTH UNTIL PAID IN FULL

Any unpaid tuition or late fees are added on to your account and must be paid in full before receiving costumes or recital tickets. Students who fall more than one month behind on payments will not be allowed to take class; until past due money is paid in full. Postdated checks will not be accepted. Tuition rates are based on a monthly class rate.

September tuition is due the first week of class

May and June tuition must be paid in cash.

Cash is accepted.

Checks are accepted, a $25.00 service charge will be charged for any returned check or payment. Please write the students name on the check if last name is different from parent/guardian.

When an assistant is available, tuition can be paid at the front desk.

FALL REGISTRATION FEES $10.00 for first immediate family member, $12.00 per immediate family. This is for all classes. There is no registration fee for summer classes.
Registration fees are non-refundable and non-transferable.

IMPORTANT! Refunds – Since tuition is a monthly fee, you will be billed from the time the student is enrolled to begin, until the end of the month. REFUNDS ARE NOT GIVEN FOR PREPAID TUITION, COSTUMES, TICKETS, RECITAL OR COMPETITION FEES. In the event, a student needs to withdraw early, a DROP CLASS FORM MUST BE COMPLETED TO CLOSE THE ACCOUNT and thus, prevent further billing. Students paid in full for a whole year tuition cannot receive a refund, but balance of payment will remain on credit for up to one year from time of cancellation. A student’s absenteeism does not affect tuition payments. Account remains open and payments due until a completed” drop class” form is turned in.
ACCOUNT WILL BE CLOSED ONLY ON THE DAY A “DROP CLASS” FORM IS RECEIVED AT THE STUDIO.

If you decide to drop a class, a “drop class” form must be filled out 30 days prior to the date you wish to drop the class or classes. If 30 days notice is not given the student will continue to be billed until a drop form is completed.

Tuition is non-refundable and non-transferable even if you or your child does not complete the month.

INDEMNITY
You agree to indemnify and hold BDS and its subsidiaries, affiliates, share holders, officers, agents, employees, partners and licensors harmless from any claim or demand, including reasonable attorneys’ fees, made by any third party.

Discounts – Full year: Pay whole Sept-June session in advance and save 10%.

Technique and stretch, only Classes – We offer technique and stretch, only, classes in ballet and jazz. These are for students wishing to increase their technical prowess in dance without the worry of preparing for a performance. These are open to all, but are required for members of the Company

STUDENTS DANCE AT THEIR OWN RISK!

INSURANCE: All students are required to be covered by their own insurance, and if an injury occurs, it is understood that the students and their family are responsible for any medical costs and not Bedazzle Dance Studio Inc., the owner, employees, heirs or assigns. All students and/or their parents must sign this release form in order to participate in classes. You dance at your own risk.

MODIFICATIONS TO SERVICE
BDS reserves the right at any time and from time to time to modify or discontinue, temporarily or permanently, the Service (or any part thereof) with or without notice. You agree that BDS shall not be liable to you or to any third party for any modification, suspension or discontinuance of the Service

CHOICE OF LAW AND FORUM
The PRR and the relationship between you and BDS shall be governed by the laws of the State of New York without regard to its conflict of law provisions. You and BDS agree to submit to the personal and exclusive jurisdiction of the courts located within the County of Richmond, New York.
STATUTE OF LIMITATIONS
You agree that regardless of any statute or law to the contrary, any claim or cause of action arising out of or related to use of the Service or the PRR must be filed within one (1) year after such claim or cause of action arose or be forever barred.

Bedazzle Dance Studio Inc. is not responsible for any lost/stolen items or injuries done on or off the premises.

SNOW & BAD WEATHER: You must call the studio at 1 – 718- 980 – 0189 for a recorded message that specifically states that we are closed. If you miss a class due to bad weather, and we are open, you are considered absent. If you call and do not get ANY message, email BedazzleDance@aol.com or try again later.

HOLIDAYS: We are open for all holidays except COLUMBUS DAY, HALLOWEEN, THANKSGIVING weekend, CHRISTMAS VACATION (check with studio for dates),WINTER BREAK, SPRING BREAK,OR EASTER VACATION (check with studio for dates), and MEMORIAL DAY WEEKEND. WE DO NOT CLOSE FOR ANY GOVERNMENT, RELIGIOUS HOLIDAYS OR SCHOOL CLOSINGS THAT ARE NOT LISTED HERE. Please make note of these dates on your calendar.

There will be NO credit issued for any sicknesses, holidays, or snow days.

BEING LATE TO CLASS – we understand that being late is sometimes unavoidable, however, a pupil who is more than 10 minutes late for class may be asked by the teacher to observe rather than take class. This is not a disciplinary measure, but it is done with the student’s best interests in mind — especially those where higher levels are concerned. A student who misses the beginning exercises runs the risk of injury by not being properly warmed up. Please make sure the student understands this.
We apologize for the strict rules on missing classes, but it is unfair to the students who work so hard to perform and do a good job in the recital. The children and the parents who insure their children are on time, and in class are impaired when another student rarely shows up, dances sloppy or makes mistakes because they don’t know the work or are not polished. Also for safety issues! We hope this helps you understand. Thank you.

STUDENTS MUST MAKE EVERY EFFORT TO BE ON TIME & ATTEND EVERY CLASS!

LEVELS & MOVING UP: Students do not move up to the next level according to the amount of years that they have studied dance. They must be able to execute proper technique for each movement to the fullest for that level. Once the teacher or director has determined that the student is ready to move up, then and only then will the student be moved to a higher level. Some students may take several years and others only one, according to natural ability, effort, and practice. POOR ATTENDANCE WILL AFFECT PROGRESSION.

OBSERVATION – Viewing windows are available sometimes throughout the year. There is no “in class” observation. Parents may be invited in to view dance routines, now and then.

BEHAVIOR: Students/parents are expected to behave in class/studio and mind the teacher at all times. Teachers will verbally discipline students, if necessary. If a student/parents becomes disruptive, conflicts with another student/parents, or doesn’t follow instructions he or she will be removed from the class/studio at the teachers’ discretion, and will not be able to attend again. A refund will not be issued to students/parents who have been removed as a result of conduct.
Appropriate behavior and language will be expected from all students, parents, and staff members.
All Students/Parents must respect and obey the teachers’/staff instructions in the studio.

There is no food, drinks, gum chewing, or smoking allowed in the dance studio.
Students are not permitted in the studio without a teacher present.

DANCE CORRECTIONS: There will be times when the teachers will employ hands-on corrections to the students for dance instructions.

Parents are not permitted in the dance room while dance is in session. Unless teacher gives permission.

Sitting down and leaning on the ballet barres is NOT permitted

ATTENDANCE: Awards are given at the end of the regular season (usually at the dress rehearsal) for good and perfect attendance. Good attendance is imperative, as absences and tardiness can hold back an entire class. Students with perfect attendance will receive an award. Perfect attendance = never miss a class and have started with the first class in September. The student will receive an award. Classes that are canceled by the studio will not count against a student’s record.

PLEASE INFORM THE STUDIO IF THE STUDENT IS TO MISS CLASS.
STUDENTS OVER THE AGE OF 8 WHO MISS MORE THAN 6 CLASSES WILL BE UNABLE TO PERFORM IN THE ANNUAL RECITAL, REGARDLESS OF PURCHASE OF COSTUME OR TICKETS.
Speak to teacher if you are you to make up missed classes.

PICTURE DAY – Photos taken are not included in your tuition and can be purchased separately. All Students must take group photos on picture day. It is at the discretion of the parents on whether or not they will purchase them.

RECITAL – The recital is in June and dress rehearsal in the same week Tickets can be purchased for (To be Announced) each in advance. Videotaping and photographs ARE NOT PERMITTED at the recital or dress rehearsal. There will be a video photographer; DVD’s can be purchased at additional costs.

Costumes are required for each routine a student performs in the recital. Nonrefundable costume money must be paid 50% in November, balance paid in full in January (studio will give exact due dates in advance).

DANCING IN RECITAL IS NOT A REQUIREMENT FOR TAKING CLASSES.
Costume fees are non-refundable and non-transferable.
Costumes must be purchased in order to perform in the recital.
Costumes will only be ordered for those who have paid the deposit. Costumes will not be given out until ALL balances are paid up to date. Recital Tickets will not be given out until ALL balances are paid in full. Students will not be able to perform in the recital until all account balances have been paid in full.

By signing this form, you give full rights to the Bedazzle Dance Studio Inc. and its staff, to use photo or video images of you or your child to use for promotional purposes of the Studio only. Photos will not be used for any other purpose. Some pictures and videos may be posted on our web site at www.bedazzledance.com

COMPETITION TEAMS – In order to be apart of our competition team, you must be invited to do so at the owner’s discretion or by audition. (Company invovles many requirements, ask for details)

DRESS REQUIREMENTS
Dress requirements are strictly enforced. Students without proper dance attire will not be able to participate in class that day. Required shoes and clothing must be purchased separately, required shoes and clothing are not included in your tuition. Shoes and clothing must be uniform. Please check for teacher preference for performance.
All Classes: Females – hair securely tied up in bun or comparable hairstyle, ONLY. Leotards and tights, or unitards. Dance shorts can only be worn with tights, Loose shirts are not permitted. Underpants not needed under tights. Strong support bras are necessary. For Lyrical ballet slippers or jazz shoes, please check for teacher preference for performance. For modern dance classes you are required to dance barefoot or with paws, please check for teacher preference for performance.
Males – tee shirt, sweatpants, shorts, tank top, stretch dance pants. Ballet White ballet shoes and white socks. Hip hop and jazz requires jazz Sneakers. Tap requires tap shoes.

No Street clothes of any sort, except for Hip Hop. Students who are improperly dressed cannot take class.
Hip Hop/ Male/Female Рany clothing style that does not inhibit movement or is considered risqu̩.
Please have all shoes clearly labeled inside with child’s name

By signing below, I state that I have read all of the above statements in their entirety, and determine that I understand, agree with and completely support all policies, rules and regulations of Bedazzle Dance Studio Inc. and state that I am solely responsible for all payments due according to the registration/enrollment form unless I turn in a Drop Class Form.
